It allows providers to monitor patients outside of the clinical setting (e.g. home) using devices that collect medical data (e.g. blood pressure or weight) and easily transfers that data to the provider.

- Your provider will let you know when to take your readings with your medical device. You should take your readings as recommended by your provider.

- When using a Bluetooth-enabled medical device, you will need a smart phone or a tablet to connect to the device and to transmit the medical data to your provider. This may be a good option if you have a smartphone and would like to view your health data in an app.

- The device is owned by the physician practice and needs to be returned when your Remote Patient Monitoring is discontinued for any reason.
- RPM Software used is HIPAA compliant and secure. Communications with your Healthcare Provider through your electronic device is not secure.
